Upaya Peningkatan Self-Management Pasien Diabetes Melitus Melalui Pemberdayaan Keluarga di Desa Sukolilo Kecamatan Sukodadi Kabupaten Lamongan

Diabetes melitus merupakan salah satu penyakit kronis yang pengelolaannya masih kurang optimal. Keberhasilan pengelolaan pasien diabetes dapat dilakukan dengan meningkatkan self-management pasien. Pengabdian masyarakat di Desa Sukolilo, Lamongan bertujuan untuk meningkatkan pelaksanaan self-management melalui pendekatan keluarga pasien diabetes melitus. Metode pelaksanaan pengabdian masyarakat pada kegiatan ini adalah melalui edukasi terpadu, pemeriksaan fisik, anamnesa dan pemeriksaan laboratorium. Kegiatan edukasi dilaksanakan oleh tim pelaksana pengabdian masyarakat yaitu dosen Fakultas Keperawatan Universitas Airlangga bekerjasama dengan Ahli Gizi Puskesmas Sukodadi Lamongan. Dosen yang memberikan materi edukasi adalah dosen yang memiliki latar belakang keilmuan dalam bidang Keperawatan Medikal Bedah yang telah memiliki sertifikat serta pengalaman penelitian dan pengabdian masyarakat terkait diabetes mellitus. Sementara untuk pemeriksaan fisik dan laboratorium serta anamnesa akan dilakukan oleh tim pengabdian masyarakat bersama dengan perawat dan petugas laboratorium dari Puskesmas Sukodadi, Lamongan. Hasil dari pengabdian masyarakat didapatkan bahwa sebanyak 27 pasien diabetes yang nemiliki gula darah puasa diatas 100 mg/dL, telah lebih mampu mengontrol gula darahnya karena mendapat bantuan pengawasan dan dukungan dari keluarga mereka. Keluarga pasien berperan penting untuk membantu dalam pemilihan serta makanan yang dikonsumsi. Selain itu, kontrol rutin ke puskesmas dan PROLANIS juga dilakukan agar pasien DM tetap dapat mengetahui nilai gula darah secara teratur.
